Work Hours/Shift
Full Time Day 
Florida Hospital Heartland
 Wauchula 

You will be responsible for:
  • Completes activity assessment of resident/family needs in relationship to care, condition and diagnosis
  • Actively involved with each patient to perform scheduled activities
  • Communicates patient care status to RN and other team members, utilizing data in
  • relation to diagnosis/condition
  • Documents appropriately on all contacts, date, time.
  • Completes activity assessments within the required time parameters
  • Demonstrates knowledge of equipment
  • Cleans, inspects, and maintains activity equipment and acquires replacements when necessary
  • Demonstrates knowledge in patient population
  • Demonstrates knowledge of age related activities considering sensory age related changes, physical limitations and disease morbidities
  • Has knowledge of and plans around assistive devices for population served
  • Participates in case conferences and care planning process within the hospitals policies and procedures
  • Maintains clinical knowledge and seeks to improve competency in area of responsibility
  • Responsible for own professional growth and development
  • Communicates, collects, reports accurate patient care data to appropriate members of the interdisciplinary team
  • Maintains accurate focus on communication
  • Participates as part of the interdisciplinary team as directed
  • Participates in performance improvement activities
  • Demonstrates commitment to the philosophy and goals of the organization
  • Plans and implements activities within fiscal parameters
  • Plans and organizes activities on a seven day basis
  • Works within the interdisciplinary team structure, coordinates, evaluates and     reevaluates
  • Other duties as assigned
Qualifications
What will you need:
  • Customer service skills
  • Excellent telephone manners
  • Legible penmanship
  • Computer skills
  • High School education or equivalent
  • BLS Certificate (within 90 days from hire date)
  •  NCCAP course or equivalent (Preferred)
    Two (2) years’ experience Activity assistant in the Health Care setting (Preferred)
  • Four (4) years’ graduate Occupational Therapist (Preferred)
  • Four (4) years’ graduate Recreational Therapist (Preferred)
  • Four (4) years’ experience as occupational therapist assistant (Preferred)
