Our 224-bed hospital includes a medical ICU, cardiovascular ICU, PCU, open heart surgery center and behavioral health. In addition, we have accredited programs in orthopedics, chest pain, and inpatient diabetes, plus an award-winning emergency department.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
  • Demonstrates the ability to provide social work/discharge planning services by utilizing appropriate planning, implementation and assessment.
  • Reviews the medical record on all referred patients for documentation of history, level of care and current needs.
  • On assigned referrals consults with physicians, patient, hospital staff, family members and/or significant others which results in timely and satisfactory resolution of identified problems and documents in patient's chart.
  • Develops discharge plans based upon thorough needs assessments, including patient/family needs, capabilities, limitations and availability of community resources as documented in patient's chart.
  • Performs screening and case finding to accomplish hospital's goals for high quality, cost effective patient care under the direction of the Director.
  • Screens for early patient identification of patients likely to need complex post-hospital care using:
    • Daily patient reports generated from Data Processing.
    • High risk screens.
    • Referrals from Case Managers.
    • Referrals from Nursing and Medical Staff.
    • Referrals from Emergency Department.
  • Receives other referrals from patient's family and friends.
  • Reviews the medical records on all referrals to identify, patient history, level of care and current needs, documented in patient's chart.
  • Seeks nursing and physician participation to evaluate current and future medical, nursing and social needs.
  • Performs a comprehensive assessment of presumptive service needs that includes assessing by activities of daily living (ADL's) and nursing and physician input.
  • Assists with and arranges referrals to outside agencies.
  • Assists with referrals to Patient Representative/Risk Management, Skilled Nursing Unit, and Chaplaincy.
  • Assists with transfers to other healthcare facilities when needed.
  • Attends skilled nursing unit and other patient care conferences as assigned by coordinator.
  • Participates in Performance Improvement Projects/teams as needed.
  • Responsible for the assignment of discharge disposition codes.
Qualifications
What you will need:
  • Minimum one-year experience in a setting related to health care
  • Acute care inpatient or long term care experience preferred
  • BSW or MSW with current licensure with the State of Texas
