You Will Be Responsible For:

•         The LPN is responsible for implementing the nursing process in accordance with the professional practice and understands and adheres to established company policies and regulatory procedures.

•         Communicate with physicians to secure/clarify orders for services.

•         Organizes a screening process to assist in identification of patients who may benefit from post-acute visits, 30,60, and 90 days evaluation.

•         Must have good rapport/communication skills with administrate staff at the SNF.

•         Documents all procedures and activities performed pertaining to medication dosing administration in the EMR.

  • Provides continuity of care by coordinating the flow of information between hospital departments and outside organizations, i.e. Home Health agencies, SNF.

  • Maintains active awareness and responsibility for billing events created in the EMR based on manual documentation of medication infusion.

  • Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self-care, nursing facility, home health, physical / psych facilities 

  • Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning

  • Provides information to physicians and the community on the benefits and alternatives home health and support service programs can provide

  • Secures physician advisor reviews on a timely basis in cases of potential denial of medical necessity for admission or continued stay. Prepares and delivers denial letters in case of non-covered admissions



What You Will Need:

  • LPN license required

  • Five years clinical experience required

  • Case Management experience required

  • BLS certification required within 30 days of hire date


Job Summary:

Responsible for timely patient flow by assigning, directing and assessing patient’s needs as well as being the telehealth hands and feet of the provider administering care.  Ensures the performance of quality patient care throughout the practice.  Facilitates telehealth visits leading coordination from healthcare providers, patient, institutions and information technology.  Responsible for establishing, building and maintaining strategic relationships between market physicians and their office staff and hospital administration as well as hospital service.  Responsible for promoting and marketing programs and services to physicians and their office staff.  Timely identification and resolution of issues that affect our Medical Staff and the effective delivery of health care.
