Senior Manager - Care Management – Case Management – Orlando

AdventHealth Orlando seeks to hire a Senior Manager who will embrace our mission to extend the healing ministry of Christ.

Facility Profile:

Located on a lush tropical campus, our flagship hospital, 1,107-bed AdventHealth Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. We are already one of the busiest hospitals in the nation, providing service excellence to more than 32,000 inpatients and 125,000 outpatients each year.

Department Profile:

The Care Management Department is located in every campus throughout the AdventHealth system. Our objective is to create a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for services to meet an individuals and family’s comprehensive needs. Care Management is an integrated department with physicians, clinical/nursing teams, ancillary departments and the revenue cycle teams. Care management meets the needs of our patients, families and significant others throughout the care continuum including the post-discharge needs.

Work Hours/Shifts:

M-F 8am-5pm

Job Summary:

The Senior Manager of Care Management (SMCM), under the general direction of the Director works to plan, organize, implement, and evaluate the medical/care management program at Florida Hospital. This includes capacity and length of stay (LOS) management, discharge planning, and social work. The SMCM works with the Director, staff, and directly with campus administration to facilitate both system-wide and campus-specific capacity and LOS initiatives. The SMCM oversees the department staff in planning, implementing, and re-evaluating the psychosocial and discharge planning needs of patients. Adheres to Florida Hospital Compliance Plan, in accordance with federal/state guidelines, accepted practice, hospital policy, accreditation bodies, and in cooperation with administration, the medical staff, and all operational hospital departments. Actively participates in assuring outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.

  • Leadership experience in care management, managed care, long-term care, home health care (Preferred)
  • Experience in discharge planning in acute care setting (Preferred)
  • Working knowledge of Microsoft PowerPoint (Preferred)
  • Working intermediate knowledge of Microsoft Excel (Preferred)
  • Bachelor’s Degree in Nursing or Health Related Field
  • Minimum (2) years of Leadership experience
  • Minimum (3) years functional health care setting
  • Preferred ACCM, CMCC, ACMA (Preferred)

Licensure, Certification, or Registration Required:

  • Current, valid State of Florida license as a Clinical Social Worker or Registered Nurse (Preferred)
  • Case Management certification – ACM (Accredited Case Manager), CCM (Certified Case Manager), RN-BC (Board Certified in Case Management) (Preferred)

Job Responsibilities:

  • Facilitates campus and system initiatives under the direction of the director to achieve capacity and LOS goals, Avoidable Delays, and other Care Management initiatives. Participates in campus- and system-hospital quality improvement initiatives in relation to general hospital business outside of Care Management.
  • Oversees management of the financial aspects of departmental operations in conjunction with the director
  • Develops and maintains liaisons with local, state, and federal resources. Applies knowledge of local, state, and federal programs to oversee and expedite discharges while assuring appropriate stewardship of healthcare resources.
  • Oversees provision of the discharge planning activities based on needs of the patient/family.
  • Provides oversight for the social work program, spanning across outpatient and inpatient to ensure a seamless transition and continuity of patient care.
  • Develops, implements, monitors, and evaluates departmental goals and objectives with campus, clinical teams, ancillary departments and physicians and ensures campus compliance with campus/system goals in cooperation with the director.
  • Develops and maintains Care Management policies and procedures consistent with general hospital policy and including any applicable federal, state and local laws (maintain up-to-date knowledge of current regulations/laws). Assist departments with creation of policies and procedures applicable to patient discharge/transfer while including interdisciplinary procedures/requirements.
  • Attend and/or lead and/or participate in department, campus, and system meetings to facilitate specified goals.
  • Completes all required Human Resources functions for the assigned work teams in a timely manner.
  • Provide, for campus, 24-hour/day, 7-day/week coverage to facilitate all responsibilities/activities mentioned herein as deemed appropriate by the Care Management Director.
  • Facilitate and assist Risk Management in the CMS Grievance and Mediation process.
