Our 224-bed hospital includes a medical ICU, cardiovascular ICU, PCU, open heart surgery center and behavioral health. In addition, we have accredited programs in orthopedics, chest pain, and inpatient diabetes, plus an award-winning emergency department.
GENERAL SUMMARY:
The Staff RN practices as a professional nurse within the limits of the Texas Nurse Practice Act assuming responsibility and accountability for a group of patients for a designated time frame utilizing skills, knowledge and the nursing process to ensure the quality of care provided and in harmony with the philosophy, goals and policies of the Nursing Department and hospital administration.
Qualifications
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
  • Assesses, develops, implements and evaluates a comprehensive plan of care through a multidisciplinary care team process in conjunction with the patient/family in internal and external settings.
  • Comprehensively assess patients' goals as well as their biophysical, psychosocial, environmental and discharge planning needs and financial status.
  • Procures services and advocates on behalf of the patient/family for scarce resources.
  • Contacts other key members of the team as needed.
  • Works collaboratively with staff members involved in care of case managed population.
  • Seeks peer consultation regarding cases that present problems/significant variances.
  • Acts as a liaison to post-hospital care providers and community health resources.
  • Manages each patient transition through the system and transfers accountability to the appropriate person/agency upon discharge.
  • Demonstrates ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and nursing policy and procedure with no more than three documented occurrences per year.
  • Observes, documents and reports patient's response to medications as evidenced in nurse's notes with no more than four omissions per quarter.
  • Intervention with patients exhibiting agitated/aggressive behavior utilizes least restrictive measures available to produce desired results.
  • Implements and maintains seclusion/restraint documents/flowsheets according to policy.
  • Ensures all admission forms related to patient rights are complete.
  • Ensures consent for psychotropic medications is obtained from the patient and teaching is complete and documented.
  • Completes problem list and initiates treatment plan on admission.
  • Ensures all Axis III diagnosis are included on Problem List.
  • Recognizes and identifies life threatening situations, responding with immediate action according to protocol with no unjustified documented variances.
  • Utilizes management skills and positive leadership to promote continuity, supervise, evaluate and review patient care activities on the unit during an eight or twelve hour shift to ensure delivery of quality care and maintenance of standards of the nursing department.
  • Organizes and plans work assignments at start of shift making appropriate assignments for all personnel on the unit based upon patient acuity and skill levels of staff members as evidenced by documented assignments recorded in the assignment books on each unit on a consistent basis and as observed by director, and feedback from nursing staff.
  • Provides constructive criticism and counseling of staff when necessary to facilitate productivity and quality of service on a consistent basis as observed by director.
  • Provides a leadership style that reflects the policies and procedures of the nursing department.
  • Assesses productivity of staff and intervenes as appropriate to decrease overtime and keep within monthly budget as reflected in monthly P&L and staffing analysis and observation of director.
  • Demonstrates the skill and judgement necessary to implement physician orders, nursing interventions and procedures as necessary for the care of the patient to provide quality care.
  • Utilizes nursing judgment in implementing physician orders in a safe and conscientious manner, notifying appropriate supervisor immediately of any concerns.
  • Documents drug allergies 100% of the time upon patient admission.
  • Monitors lab data and diagnostic tests received on assigned patients as observed by head nurse, charge nurse and feedback from physicians.
  • Documentation includes evidence of appropriate patient/significant other teaching and their understanding of the instruction to assigned patient.
  • Compassionately give personal patient care with consideration for physiological and psychosocial needs.
  • Effectively communicates accurate thorough patient information to peers and other health care workers as evidenced by evaluation of report or feedback from peers, supervisor and other health care workers.
  • When applicable, follows hospital protocol and physician orders regarding use of restraints.
  • Directs and collaborates with the multidisciplinary care team to ensure complete and appropriate individualization patient care.
  • Uses the results of quality of care activities to initiate charges through collaboration with associated disciplines as evidenced by multidisciplinary feedback.
  • Provides for clean and safe patient environment and well stocked nursing unit free of safety hazards by end of shift with no more than one variance per month as identified by observation by peers and director with no more than two documented concerns.
  • Oversees and coordinates patient care delivered by the multidisciplinary care team.
  • Assesses and evaluates diagnostic testing and clinical procedures performed.
  • Identifies and monitors clinical risk issues.
  • Reports all variances to appropriate resources.
  • Communicates on an on-going basis between patient/family/medical staff/multidisciplinary care team and third-party payors as necessary.
  • Relates utilization assessments needed to justify continued hospitalization.
  • Collaborates with appropriate resources for optimal reimbursement.
  • Formulates, implements and evaluates educational strategies for patient/family.
  • Educates patient/family regarding integrated case management and the roles of the multidisciplinary care team.
  • Facilitates for/provides necessary patient/education pre-admission, during length of hospital stay, and post-hospital discharge as indicated.
  • Educates patient/family regarding individualized critical pathway.
  • Contributes to the professional development of self, peers, colleagues and others.
  • Shares knowledge and skills with colleagues and others.
  • Provides peers with constructive feedback regarding their practice.
  • Contributes to an environment that is conclusive to clinical education of nursing students, as appropriate.
  • Uses interventions substantiated by research as appropriate to the individual's position/practice environment.
  • Conducts nursing practice on behalf of patient/family in an ethical manner.
  • Practices nursing as guided by the Code for Nurses.
  • Delivers care in a non-judgmental and nondiscriminatory manner that is sensitive to patient/family diversity.
  • Delivers care in manner that preserves/protects client autonomy, dignity and rights.
  • Seeks available resources to help formulate ethical decisions.
  • Assigns tasks for delegates care based on the needs of the client and the knowledge and skills of the provider selected.
  • Evaluates own nursing practice in relation to professional practice standards and relevant statutes and regulations.
  • Engages in performance appraisal on a regular basis, identifying areas for professional/practice development.
  • Seeks constructive feedback regarding own practice.
  • Takes action to achieve goals identified during performance appraisal.
  • Participates in peer evaluation when requested.
  • Associate remains flexible to changing systems, is expected to demonstrated quality and effectiveness in work habits and treat staff, physicians, patients and families with consideration and respect.
  • Understands the Computerized Physician Order entry process and proficiently uses the electronic medical record for retrieval of patient information.
  • Understands computer downtime procedures.
  • Monitors Electronic Medical Record for new physician orders and diagnostic results.
  • Performs all duties relating to Real Time Scanning accurately and efficiently.
  • Performs all other duties as assigned.
