You Will Be Responsible For:

  • Coordinates care in collaboration with the Palliative care team to ensure that patients have the community resources to be successful post discharge.
  • Ensures that patients understand and have the ability to make post hospital appointments.
  • Coordinates with the patients’ physicians and post-acute Palliative/Hospice providers to ensure continuity of care once patient is discharged.
  • Provides support and education to targeted patient populations, i.e., Palliative and/or Hospice appropriate.
  • Works closely with Palliative Care team, local Hospice providers, physicians, and nurses to assure the appropriate post discharge plan is in place for Palliative and Hospice patients.


Qualifications
What You Will Need:
  • Recent hospital nursing experience
  • Knowledgeable of current care practices for the high risk diagnoses as identified by CMS.
  • Ability to effectively communicate with patients and skilled in assessment of educational needs.
  • Demonstrates effective communication with various members of the healthcare team including staff, physicians, post acute providers, and community resource liaisons.
  • Knowledgeable regarding availability of community resources.
  • Knowledgeable of regulatory standards.
  • Knowledgeable of Corporate Accountabilites
  • Ability to communicate effectively and present information to both large and small groups
  • Ability to manage stresses and handles challenging situations and personalities.
  • Computer knowledge and skills       
  • Graduate of accredited school of nursing
  • Recent nursing experience in acute hospital environment
  • RN must have an active Florida License
Job Summary:
 

The goal of the Palliative Care Navigator is to ensure that high-risk chronically ill patients are appropriately aligned with the most effective post-acute services in an effort to improve post hospital outcomes and patients’ abilities to manage their chronic illness(es).  This will include, but is not limited to home healthcare, palliative care, patient education, primary medical care, skilled nursing, support groups, and community resources as needed to improve the overall health and wellbeing of chronically ill patients being discharged from AdventHealth Zephyrhills.
