Referral Coordinator

JOB SUMMARY

The Referral Coordinator is responsible for receiving referral requests, obtaining physician approval, processing medical records requests pertaining to referrals and processing the referral.  Schedules appointments with the referral when necessary.  Serves as liaison between patient and medical support staff with regards to referrals.

Our team continuously works with the quality team and practice coaches to implement and maintain NCQA/PCMH standards.


•    Participates in educational sessions to improve skills as appropriate

•    Participates in the collection of quality data

•    Assists clinical staff providers with providing patients with outside resources and materials

•    Assists in identifying and documenting cultural and linguistic needs of patients

•    Assists in identifying and documenting advanced directives

•    Assists clinical staff and providers with obtaining ER and Hospital information

•    Assists in arranging follow up appointments as indicated by clinical staff and providers

PHYSICAL QUALIFICATIONS

Must be able to sit, walk, climb, stand, turn, twist, bend, stoop, kneel, reach, lift at least 25 lbs., carry, finger, hold, talk, see with a good field of vision, and hear; Communicate on the telephone and directly with patients; read paperwork, printouts and computer screens; Requires manual dexterity sufficient to maintain files, operate a keyboard, calculator, telephone, copier and other small desk equipment and computer.

PERFORMANCE REQUIREMENTS

Knowledge of business office procedures.  Knowledge of English grammar, spelling and punctuation to type patient information.  Skill in operating a computer and photocopy machine.  Skill in greeting patients and answering the telephone in a professional, pleasant and helpful manner.  Ability to speak clearly and concisely.  Ability to read, understand, and follow oral and written instruction.  Ability to type 40 words per minute accurately.  Ability to establish and maintain effective working relationships with patients, employees and public.

 

JOB QUALIFICATIONS

1.   High school graduate or equivalent.

2.   One year of ICD-9 and CPT-4 coding experience preferred.

3.   Experience with computers is required.

4.   Must be willing to float to other AHMG practices, within reason, when patient load, vacation schedules etc. make it necessary.

5.   Demonstrates ability to communicate by reading, writing legibly, speaking and comprehending English effectively in order to carry out job requirements.
