Program Manager – Population Health – Maitland

Florida Hospital seeks to hire a Program Manager who will embrace our mission to extend the healing ministry of Christ.

Job Summary:

The Population Health Operations Manager is responsible for ensuring operational readiness and sustainability of new Population Health assets. The Operations Manager works closely with IHS senior leadership and service line leaders to manage Population Health Operations according to organizationally determined accountabilities. The Operations Manager works closely with IHS leadership in consulting and developing deployment and operating plans for the growth initiatives identified for IHS. This includes but is not limited to operational dashboards, executive summaries, and other forms of communication to serve as updates to key stakeholders. The Operations Manager will manage strategic initiatives to achieve the stated growth plans for IHS. Develops work plans and helps to hold departments accountable to ensure timely completion of initiatives. Oversees Analytics and Episode Payment Model functions and team members. Collaborates with various departments across FH to collect feedback and support. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.



Knowledge, Skills, Education, & Experience Required:

  • Strong organizational skills, self-directed, and ability to manage multiple priorities/projects
  • Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of leadership, physicians, and staff
  • Proficiency in Microsoft Office Suite
  • Highly skilled public speaker with ability to effectively communicate to small and large groups
  • Excellent communication, interpersonal skills and ability to lead, facilitate, consult, research and educate
  • Ability to manage and work with a diverse group of individuals, while striving for excellence
  • Ability to accept responsibility for actions and results, with a drive to achieve and outperform, while continuously looking for improvements
  • Problem solving skills with the ability to think creatively
  • Ability to be a change agent who positively influences others to gain acceptance for important System and clinical programs and processes
  • Knowledge of clinical operations; safety, regulations, processes, finance
  • Strong skills in quickly developing relationships with the ability to influence and change behaviors through evidence-based practices.
  • Analytical thinking and highly skilled in working with data and technology
  • Bachelor’s degree in Business Administration, Healthcare or related field
  • 2 years of experience in healthcare, or project management
  • 3 years of experience in business development, operations, workplace wellness or population health
  • Master’s degree in Business Administration, Healthcare, or related field (Preferred)
  • 3 years of experience in business development, operations, workplace wellness or population health (Preferred)

Job Responsibilities:

  • Responsible for operational readiness and sustainability for all new and redeveloped Population Health assets. Ensures each asset is delivered on time, and works closely with finance to track the actual spend compared to budget.
  • Oversees and is constantly aware of the status of each initiative. Anticipates potential roadblocks that may affect completion and communicates issues with Leader. Completes documentation for all phases of the project, in accordance with the Population Health governance process.
  • Accountable for creation of comprehensive work plans. Responsible for all communication throughout the development life cycle of new or existing assets. Communication format may be verbal, written, or in presentation format and may include communicating performance updates to Executive and Senior Leadership.
  • Manages analytics and episode payment models functions and team members, along with multiple priorities as required and balances work by taking ownership of operational planning tasks related to individual projects under the direction of Senior Leadership.
  • Manages, participates in, and facilitates project committees in the accomplishment of IHS Population Health goals by arranging meetings, agendas, presentations, process flows, minutes and applicable project data. Manages logistics and coordinates events.
  • Assists in the identification, analysis and resolution of system or process problems and quickly collects data to establish facts in order to determine the cause of these problems. Tracks and follows-up on issues then contacts and works with appropriate operational staff to resolve issues. Assimilates past experiences and job knowledge to solve problems in a timely manner. Works closely with the Executive Director and leads team members in trouble-shooting and/or problem solving.
  • Collaborates with IHS departments to ensure strategic and operational requirements are incorporated and appropriately executed, such as working closely with Florida Hospital Medical Group and other ambulatory entities.
