You Will Be Responsible For:

  • Demonstrates expert level functional knowledge of one or more of the assigned applications.
  • Prepares training documentation and provides education on new applications or functionality. 
  • Conducts train-the-trainer, super-user, and optimization with moderate direction from the application manager. 
  • Demonstrates basic knowledge of operations within the department.          
  • Educates on Population Health applications for unique ambulatory provider/practice scenarios Serves as subject matter expert on Population Health PHSO programs.


Qualifications

What You Will Need:
  • College Degree (AS, B.A., B.S. or similar) in a field of study such as business, IS, education, or a healthcare related area
  • Yearly participation in ongoing education either formal or through conferences, active professional organization participation, or journal presentations.
  • 4+ years’ experience working collegially with physicians
  • Basic knowledge of affiliate/community ambulatory provider practice operations and services impacted by managed care and health care reform
  • Basic knowledge of processes within the ambulatory practice/provider setting, to include workflows and efficiencies required for a successful practice operation
  • Demonstrated experience in preparing and delivering reports
  • Demonstrated Electronic Health Record (EHR) experience to include either eClinicalWorks , Athena, etc.
  • Experience with public speaking to include basic presentation skills
 
Job Summary:

This position will act as a Population Health subject matter expert, providing consultation and education to physicians.  The educator will create training materials, communication, and workflows to provide one on one and classroom education sessions.

Population Management Educators are expected to make regular visits to physician offices, with the intent to educate on a wide variety of topics such as population health management strategies, clinical data integration, Electronic Health Records, and Population Management tools. 

Educators will perform duties with moderate direction from their manager. Key to success is the ability to recommend workflow and systems education to ambulatory practices. Educators must be able to articulate complex topics in a way which facilitates understanding, both to providers as well as other Population Health Services Organization and Information Technology teams.

Educators will function as subject matter experts to various audiences through communication and educational initiatives to connect key concepts and expectations, ultimately presenting the technologies, educational opportunities, and efficiencies in a manner that is simple for providers to relate to and drives value to the ambulatory practice.
