The largest health care provider in Johnson County, Kansas, the SMH network includes SMMC and SMH - Prairie Star and will soon include SMH - Blue Valley, a third major health care facility. Our 54-acre campus at SMMC is comprised of the main hospital, a community health education building, six physician office buildings and an associate child care center. The Emergency Department at SMMC is the busiest in Johnson County. We deliver more babies each year at the Shawnee Mission Birth Center than any other hospital in the metropolitan area and our Center for Women’s Health is nationally recognized. We have seven Shawnee Mission Primary Care locations with more than 30 Board-certified doctors who specialize in family medicine, internal medicine and pediatrics as well as Centra Care Shawnee Mission Urgent Care locations.

You will be responsible for:

  • Delegates and/or assigns nursing care responsibilities considering the following factors - the degree of supervision needed by the associate, competence and experience level, staff availability, and patient acuity.
  • Utilizes Patient Care Problem Solving Process and/or Conflict Resolution Process to resolve patient care conflicts.
  • Initiates and maintains accurate records, appropriate legal documents and other healthcare reports.
  • Transmits/prescribes prescription orders pursuant to written protocols as authorized by a sponsoring physician and in accordance with DEA requirements.
  • Incorporates patient, physician, patient needs and concerns into decision-making and organizational action.
  • Performs initial and on-going assessments of self-care, psychosocial needs, education, environment and discharge planning, incorporating information from the identified significant other and other health providers.
  • Utilizes skills of observation, inspection, palpation, percussion and auscultation using diagnostic instruments or laboratory procedures that are basic to the screening of physical signs and symptoms.
  • Assesses learning needs, readiness, and capabilities of patient/family, including cultural, religious practices, language barriers, emotional barriers, educational level, motivation and limitations.
  • Documents patient care findings within established standards utilizing CPOE.
  • Seeks appropriate resources to clarify information concerning the nursing/medical care regime as needed.  Coordinates care by collaborating with physicians and others to obtain appropriate services, including multidisciplinary team meetings.
  • Provides an accurate, thorough and timely report of patient’s condition to physicians and multidisciplinary team members. Works collaboratively with Utilization Review and Social Services regarding patient’s LOS and discharge planning.
  • Initiates the multidisciplinary teaching plan which provides information to patient/ family/ significant other in the areas of (as appropriate): the safe and effective use of medications, nutrition/modified diets, potential food/drug interactions, safe and effective use of equipment, rehabilitative techniques, access to community resources and when and how to obtain further treatment.

  • Communicates the plan of care across the continuum by thoroughly completing appropriate transfer forms and discharge summary forms.
  • Selects therapeutic interventions which are appropriate to the patient’s needs and desired outcomes. 
  • Collaborates with external agencies that our patients are referred and provides in-service programs to help ensure patient’s plan of care is extended for the next phase of care.
  • Manages the use of supplies/equipment in an efficient manner. 
  • Ensures that patient’s medical record has appropriate documentation to meet all quality and patient experience indicators.
  • Utilizes hospital pharmacy formulary.
  • Works with interdisciplinary teams to reduce un necessary readmissions to the hospital.
  • Priorities work day to address timely consults, discharge planning and quick response.
  • Actively participates in department Meetings, quality improvement meetings, and staff education as requested.
What will you need?
  • Master’s Degree
  • Up to 1 Year of Position-Related Experience
  • Kansas Physician Assistant License
  • 5+ Years of Position-Related Experience
