Work Hours/Shift:

You Will Be Responsible For:
  • Ensures that the patient’s plan of care includes all CMS, JCAHO, GWTG, Action Registry and all of the quality/outcome data base indicators and accurate and complete documentation is reflected in each patient’s medical record.
  • Seeks appropriate resources to clarify information concerning the nursing/medical care regime as needed.  Coordinates care by collaborating with physicians and others to obtain appropriate services, including multidisciplinary team meetings.
  • Provides an accurate, thorough and timely report of patient’s condition to physicians and multidisciplinary team members. Works collaboratively with Utilization Review and Social Services regarding patient’s LOS and discharge planning.
  • Initiates the multidisciplinary teaching plan which provides information to patient/ family/ significant other in the areas of (as appropriate): the safe and effective use of medications, nutrition/modified diets, potential food/drug interactions, safe and effective use of equipment, rehabilitative techniques, access to community resources and when and how to obtain further treatment.
  • Initiates the multidisciplinary teaching plan which provides patient/family/significant other skills to address, as appropriate; the safe and effective use of medications, nutrition/modified diets, potential food/drug interactions, safe and effective use of equipment, rehabilitative techniques, access to community resources and when and how to obtain further treatment.
  • Communicates the plan of care across the continuum by thoroughly completing appropriate transfer forms and discharge summary forms.
  • Selects therapeutic interventions which are appropriate to the patient’s needs and desired outcomes.  

Qualifications
What You Will Need:
  • Master’s Degree
  • Up to 1 Year of Position-Related Experience
  • Kansas Physician Assistant License
  • BCLS obtained through the American Heart Association
 
Job Summary:

 The Physician Assistant is a professional mid-level provider licensed in Kansas to function in an expanded role to provide primary health care to individuals, families or groups.   The Physician Assistant functions in a collegial relationship with physicians and other health professionals in the delivery of health care services. Provides care for patients of the practice within the scope of licensure and within the supervising Physician’s scope of practice.  They are able to make independent decisions with physicians in carrying out health regimens for families and clients.   The Advanced Practice Provider demonstrates AIDET, is proficient in CPOE and is a resource to the physicians, nurses and other disciplines.  In collaboration with the physician, the Physician Assistant is able to perform a comprehensive History and Physical, develop and implement a plan of care, order and interpret results of laboratory and diagnostic tests, coordinate and initiate consults, review and adjust medications and dosages, provide patient education and comprehensive discharge planning.  The Physician Assistant consults and collaborates with other physicians and disciplines to ensure smooth and timely throughput, including coordination of discharges and transfers.  The Physician Assistant documents and meets criteria for evidence based and quality outcomes.  The Physician Assistant may work closely with utilization review, multidisciplinary teams and floor managers to achieve prompt and proper admission and placement, optimize length of stay, and provides superior quality service.
