Work Hours/Shift:

Full Time / Days

You Will Be Responsible For:

1.       Develop and execute a structured orientation for personnel as assigned.

2.       Provide level of care assessments, individual, family, and group therapies as assigned.

3.       Provide staff supervision and feedback to clinical social workers as assigned.

4.       Utilizes evidence-based interventions to assist patients, families, and significant others in coping with the psychosocial aspects of illness and its treatment.

5.       Develops discharge plans in collaboration with physicians, nurses, physical therapists, nutritionists, administrators, educators, community agencies and others.

6.       Develops, executes, and monitors completion of Intake and Assessment level of care assessments are completed in an accurate and timely manner,

7.       Consults with physicians, patient, hospital staff, family members or significant others in a timely manner to facilitate resolution of identified barriers to treatment progress to ensure the continuity of care.

8.       Maintains pertinent records of social work intervention, assessment, diagnostic impression and treatment for use in compiling accurate statistics.

9.       Coordinate referrals to federal, state, county and community agencies in a timely manner. Develops working relationship with agencies to ensure appropriate service delivery to patients.

10.    Assesses patient for behavioral health diagnoses according to the DSM-V as needed. Provides appropriate referrals for behavioral services as needed.

11.    Identifies social determinants in patients that may contribute to treatment problems. Identifies and provides info about community resources from which patients may benefit.

12.    Ability to recognize, identifies, and communicates potential abuse and neglect issues with Risk Management, patients and family members. Effectively communicate with Adult and Child Protective Services to ensure continuum of care during and post hospital stay.

Qualifications

 What You Will Need:


•     Requires graduation from accredited Master’s Degree program, School of Social Work.

•     Requires knowledge of the principles, laws, and practices of social work.

•     Master’s Degree program, School of Social Work

•     Appropriate licensure as required by state law.

•    At least two (2) years of experience as a clinical social worker in a behavioral health hospital setting is preferred.


•    Experience should include level of care assessments, individual, group, and family therapy and aftercare plans post hospitalization in an acute care psychiatric setting (preferred).


•     Advanced Clinical Practitioners License (preferred).


•   Ability to complete and maintain certification in Crisis Prevention Institute (CPI) behavioral patient management course (preferred).


•     Ability to complete and maintain BLS certification (preferred).

Job Summary:

The Behavioral Health Center (BHC) Clinical Program Coordinator is responsible for day-to-day supervision and guidance of Intake and Assessment personnel and patient experience; managing intake staffing; responsible for timely and accurate completion of screening, assessment, and community referrals for BHC and Hospital. Assists in developing, coordinating and assists in staff education. This position is responsible for duties and/or projects assigned by Manager. This position is responsible for day-to-day supervision of the inpatient Clinical Social Workers in the absence of the manager. This position reports to the Clinical Program Manager.

Knowledge and Skills Required:     

Department, facility and system-wide responsibilities, which may include reporting responsibilities to Medical Staff and Executive Committees. Representative and liaison for facility on system-wide committee and task forces. Customers are internal and external with BHC staff, managers, executives, licensing and regulatory agencies, third party payers and contractors.
