You Will Be Responsible For:

  • Punctual with returning pages and phone calls, with completing documentation, timeliness in arriving at meetings and for patient treatments.
  • Respects each discipline's expertise and knowledge; doesn't question someone's expertise in front of the patient/family.
  • Professionally communicates with patients, their families, their physicians, and referral sources.
  • Respects authority by following the appropriate channels of communication in resolving problems and presenting information.
  • Readily accessible for new patient assignments.
  • Demonstrates flexibility in accepting patient assignments.
  • Communicates changes in the patient's condition with the Case Manager or the Supervisor for effective Case Management.
  • Completes and maintains clinical records in a timely manner and in accordance with the agency's policy.
  • Demonstrates the ability to perform the PT Assessment, effectively communicating the results with the entire team, including the physician.  
  • Performs the PT re-assessment as needed and at recertification within the established timeframes.
  • Notifies patient's physician of pertinent changes.
  • Notifies patient's physician of discharge plans.
  • Processes add-on orders in a timely manner.
  • Completes the HHAide Supervisory visits at least biweekly.
  • Regularly checks and updates the HHAide assignment sheet.
  • Communicates on a daily basis with supervisor, also communicating schedule changes as they arise.
  • Attends orientation, in-services, and meetings, as scheduled.
  • Delegates and supervises appropriate patient care duties performed by Physical Therapy Assistants, Home Health Aides, and students.
  • Obtains physician's written prescription and approval for treatment changes.
  • Complies with established PT practices and standards of care.
  • Incorporates patient, physician, customer needs and concerns into decision-making and organizational action.
  • Provides complete patient care from initial evaluation through discharge with instruction for pre- and post-discharge care in accordance with department practice, policies and procedures.
  • Performs evaluations as appropriate to diagnosis.
  • Interprets findings to patient, family and physician.
  • Responsible to educate patients and/or their families in self-care, prevention, body mechanics, life style adaptation, posture, and ergonomics as appropriate.  
  • Develops and implements treatment using appropriate techniques within physician's prescription, obtaining approval for changes.
  • Completes evaluation/progress/discharge notes in a timely fashion and within agency guidelines.
  • Communicates in a timely manner with the physician and other referral sources.
  • Discusses discharge plans of assigned patients with the team, patients and their families.
  • Participates on the patient care team in order to provide comprehensive care and continuity of services.
  • Supervises and plans HHAide services appropriate to the individual patient's requirements and in accordance with department policy.
  • Provides weekend and holiday coverage as assigned by the patient care supervisor.
  • Participates in student supervision as indicated.
  • Demonstrates the initiative to learn and applies new processes and procedures by attending in-services, pertinent continuing education seminars, and reading profession journals.
  • Provides educational in-services to the clinical staff after attendance at agency seminars/workshops.
  • Participates in planning programs for the agency and the patients.
  • Participates on agency committees as appropriate.
  • Participates constructively in department in-services, meetings, and activities.  
  • Renders services in accordance with PT Standards of Practice.
  • Discusses new cases with Supervisor within 24 hours of admission and at least biweekly throughout the period of care.
  • Communicates with team members for the effective implementation of the plan of treatment.
  • Provides licensure, insurance information, and CPR certification, as requested.
  • Mentors new employees, as requested.  
  • Participates in Quarterly Chart Audits.
  • Documents patient supplies on Agency Supply Closet check-out sheet, in daily notes, and on verbal orders.
  • Demonstrates the ability to organize workload in order to meet productivity standards.
  • Complete initial documentation within 24 hours.
  • Processes add-on orders within 24 hours.
  • Follows frequency and duration outlined on initial orders/485.
  • Completes documentation accurately the first time.
  • Responsibly cares for tools/equipment issued by agency.

What You Will Need:
  • Kansas Physical Therapy License
  • Missouri Physical Therapy License
  • BLS obtained through the American Heart Association
  • Valid Driver’s License

Job Summary:

This position is accountable for the evaluation, planning and implementation of patient care activities, as prescribed by a physician.  This individual is a member of a team of multidisciplinary health care professionals who contributes to individual patient care plans in order to maximize function within the home, restoring health, and prevention of  injury or illness.  This person may supervise and responsibly direct the activities of PTA's and Home Health Aides, in the provision of care.  Patient treatments are provided in a professional manner and within the scope of established home care guidelines and standards of practice.  Documentation is thorough and completed in a timely manner. This individual works directly with Physicians, home care Nurses, Physical Therapists, Occupational Therapists, Speech and Language Pathologists, Social Workers, PTA's, Home Health Aides, office staff and supervisors.  Customer Service is at the center of this position.
