Director Care Coordination AdventHealth West Florida Division

Work Hours/Shift:

Full Time/Days

You Will Be Responsible For:

  • In collaboration with WFD Post-Acute and  PPN executive leadership establishes infrastructure within the local market for the PPN, including collaborative forums, performance metrics, measurement and reporting methodologies and frequency, methodology for education and mentoring of PPN members to establish and maintain evidence-based post-acute care.
  • Establish tools and programs to integrate post-acute care providers, including universal discharge packets and care transitions processes that cross the acute to post-acute continuum.
  • Establishes initial and ongoing communications regarding the PPN: throughout local AH hospitals, with affiliated and unaffiliated physicians, and through the marketing and communications department to assure that patients, families, physicians, health professionals, community organizations and the public-at-large are made aware of the AH PPN, PPN members and their qualifications.
  • Works with case managers leaders/ discharge planners/hospitalists appropriate language to assist patients and families to make informed decisions regarding quality post-acute providers.
  • Develops physician relationships to drive utilization of the PPN and addresses barriers to physician documentation and other inhibitors of maximizing value of the PPN.
  • Serve as the West Florida lead contact for transitions of care trends and issues to bring to post-acute entity leadership for process improvements.
  • Lead for CM Review of patient cases for transfers into our region. 
  • Prepares monthly and quarterly reports for distribution to regional, facility and corporate leadership.


Qualifications

What You Will Need:

EDUCATION AND EXPERIENCE REQUIRED:

  • Registered Nurse with a Bachelor of Science in Nursing
  •  MSN or Master Degree in Healthcare Services Administration  
  • Registered Nurse: Minimum of ten years of clinical nursing experience in an acute care hospital.
  • Minimum of five years of leadership experience in the field of Case Management/Utilization Review, and Social Services/Social Work. Department(s) may include but not limited to: Director of Resource Management, Director of Care Coordination, or Director of Case Management and Social Services.

EDUCATION AND EXPERIENCE PREFERRED:

  • CDI and or ICD-10 training, and or high level knowledge of coding principles in the hospital inpatient and outpatient settings.
  • Case Management leadership experience in a multihospital system, community-based, rural-based, and academic hospital settings

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • Current Florida RN required
  • Certification in Case Management Standards required; ACM or CCM obtained within 12 months of hire.

Job Summary:

The Director of Care Coordination is a multi-faceted role that reports to the West Florida Division Senior Executive Officer of Patient Outcomes. 

The Director of Care Coordination directs, maintains and evaluates the West Florida Division Preferred Provider Network (PPN) for skilled nursing facilities (SNFs) and home health agencies (HHAs); will oversee the Care Transformation activities for the West Florida Division; and will oversee Bundle programs for the West Florida Division.

Develops and leads integration of unaffiliated and affiliated post-acute providers with the goal of an acute/post-acute seamless care continuum for AdventHealth patients.  Responsibilities include the partnership with all of the post-acute entities in the region including SNFs, HHAs, Hospice programs, Long Term Acute Care (LTAC) and Inpatient Rehabilitation Facilities (IRFs) to promote and maintain Quality Improvement activities to support safer and more efficient patient transitions. This role directs, develops, maintains and evaluates the operations of Bundle Program patients in the West Florida Division.  The Director of Care Coordination will work in collaboration with the Care Navigation teams to connect patients to post-hospital follow up care and providers.  Responsible for the oversight of Care Transformation, working with the clinical leadership to coordinate the execution of process changes.  The Director facilitates collaborative relationships with the West Florida Division CFOs, CNOs, CMOs, Care Management, Nursing, and AdventHealth leaders to optimize patient care coordination throughout the West Florida Division.
