You Will Be Responsible For:
  • Provide clinical pharmacy services, including primary and consultative services, to health care providers and patients to ensure optimal drug utilization in assigned specialty area(s).
  • Performs clinical consults on glycemic management, anticoagulation, kinetics, TPNs, renal dosing, IV to POs, antimicrobial stewardship, COPD and pain management.
  • Assist in the development, implementation and continued maintenance of programs like residency (PGY1), pain management/ OR services, ED pharmacy services (responding to stroke alerts and codes, performing medication history evaluation, verifying ED orders)
  • Assist in the planning and development of clinical pharmacy activities related to specialty area(s).
  • Participate in clinical research, MUEs, and committees (i.e., P&T, VAT) as assigned.
  • Provide education and training to pharmacy students, medical staff physicians and nurses, and healthcare-related professionals. 
  • Participate in interdisciplinary patient care rounds and communicate follow-up with clinical team.
  • Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team.

What You Will Need:
  • Perform Clinical Consultations on Glycemic Management, Anticoagulation and Pharmacokinetics
  • Graduate of an AACP accredited pharmacy school
  • State of Florida License in Registered Pharmacist
  • Clinical Pharmacy expertise in TPN, Antimicrobial Stewardship, ED Services Pain Management/ OR Services (preferred)
  • Pharm. D or Residency (preferred)
  • Board Certified Pharmacotherapy Specialist (preferred)
  • State of Florida License in Consultant Pharmacist (preferred)
Job Summary:

The Clinical Pharmacist – Residency Trained shall support the mission of the hospital and pharmacy department by preventing, identifying and resolving drug related problems to assure patient safety, improve patient satisfaction and minimize costs. The Residency Trained Clinical Pharmacist is responsible for developing and providing clinical pharmacy services directed to the general patient population at Florida Hospital Heartland Division and Wauchula.
