PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:

Provide clinical pharmacy services, including primary and consultative services, to health care providers and patients to ensure optimal drug utilization in assigned specialty area(s).
Performs clinical consults on glycemic management, anticoagulation, kinetics, TPNs, renal dosing, IV to POs, antimicrobial stewardship, COPD and pain management.
Assist in the development, implementation and continued maintenance of programs like residency (PGY1), pain management/ OR services, ED pharmacy services (responding to stroke alerts and codes, performing medication history evaluation, verifying ED orders)
Assist in the planning and development of clinical pharmacy activities related to specialty area(s).
Participate in clinical research, MUEs, and committees (i.e., P&T, VAT) as assigned. 
Provide education and training to pharmacy students, medical staff physicians and nurses, and healthcare-related professionals.  
Participate in interdisciplinary patient care rounds and communicate follow-up with clinical team.
Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team.
Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, dosage form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
Appropriately document clinical interventions.
Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
Participate with Clinical Coordinator/Director of Pharmacy/Quality Director, clinical team and the Pharmacy department to evaluate and measure the impact of new approaches that have been implemented.
Participates with Clinical Coordinator/Director of Pharmacy to identify high-cost and high-use agents within defined area of practice and participates in financial management initiatives (revenue generating, cost containment, etc).
Report medication errors and adverse drug reactions.
Ability to supervise and train assigned staff. 
Generates reports for review of drugs used in treatment of ADR's.
Performs therapeutic drug level monitoring when requested, using department kinetic software.
Prepares adverse drug reaction reports when suspecting an ADR.
Provides drug information upon request.
Responsible for closure/follow-up of own clinical notes.
Works cooperatively with all members of the work team.
Report medication errors, near misses, ADR's problematic events in Risk Master.
Demonstrate knowledge and adhere to USP 797 standards.
Demonstrate knowledge of JC standards and NPSG's.
Perform all other duties as assigned.
Qualifications
KNOWLEDGE AND SKILLS REQUIRED:

Perform Clinical Consultations on Glycemic Management, Anticoagulation and Pharmacokinetics

KNOWLEDGE AND SKILLS PREFERRED:

Clinical Pharmacy expertise in TPN, Antimicrobial Stewardship, ED Services, Pain Management/ OR Services

EDUCATION AND EXPERIENCE REQUIRED:

Graduate of an AACP accredited pharmacy school

EDUCATION AND EXPERIENCE PREFERRED:

Pharm. D or Residency
Board Certified Pharmacotherapy Specialist

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

State of Florida License in Registered Pharmacist

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

State of Florida License in Consultant Pharmacist
