You will be responsible for:
  • Provide clinical pharmacy services, including primary and consultative services, to health care providers and patients to ensure optimal drug utilization in assigned specialty area(s).
  • Assist in the planning and development of clinical pharmacy activities related to specialty area(s).
  • Participate in clinical research as assigned.
  • Responsible for compounding and dispensing medications and parenteral admixtures in accordance with established Federal and State regulations and accreditation standards.
  • Effectively verify patient medication orders for appropriateness and accuracy.
  • Provide education and training to pharmacy students, medical staff physicians and nurses, and healthcare-related professionals. 
  • Prioritize level of care based on critical needs.
  • Participate in interdisciplinary patient care rounds and communicate follow-up with clinical team.
  • Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team.
  • Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, dosage form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
  • Appropriately document clinical interventions.
  • Appropriately complete Interdisciplinary Plans of Care (IPOC) as assigned.
  • Participate in opportunities to improve the quality, effectiveness, and/or efficiency in distributing medications to our customers and patients.
  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
  • Provide and document discharge medication teaching upon request.
  • Participate with Clinical Manager, clinical team and the Pharmacy department to evaluate and measure the impact of new approaches that have been implemented.
  • Participates with Clinical Manager to identify high-cost and high-use agents within defined area of practice and participates in financial management initiatives (revenue generating, cost containment, etc).
  • Report medication errors and adverse drug reactions.
  • Ability to supervise and train assigned staff.
  • Perform all other duties as assigned.
  • Support the Mission, Vision and Values of the Department of Pharmacy.
  • Support Florida Hospital Tampa Core Values of Integrity, Compassion, Balance, Excellence, Stewardship, and Teamwork (ICBEST).
  • Provide clinical pharmacy services directed to the general population at Florida Hospital Tampa
  • Provides at least one (1) Pharmacy staff development initiative per year.
  • The pharmacist shall provide accurate and clinically relevant drug information to physicians, nurses, and other health care providers upon request in accordance with their expertise to do so.
  • Participates in mentorship and professional development of pharmacists, technicians, pharmacy residents, and pharmacy students.
  • Participate collegially in the development of new programs, services, and practices, educational activities of the pharmacy department and medical team.
  • Consistently attend pharmacy student/residency seminars and presentations.
  • Adhere to the Florida Hospital Tampa formulary by assisting in recommending formulary changes to physicians and developing clinical programs to promote appropriate, cost-effective prescribing.
  • Actively participates in the development and implementation of performance/quality improvement and patient safety activities for clinical service area, Pharmacy, and Florida Hospital Tampa.
  • Continue to grow in the role of a Florida Hospital Tampa clinical pharmacist.
  • Design and assist in Provider education and drug awareness programs, provide clinical support to the Pharmaceutical Services team.
What will you need?
  • Fully understands and can apply all state and federal laws and regulations related to the practice of pharmacy.
  • Must possess excellent knowledge of current drug therapies, drug interactions, pharmacokinetics, and sociologic principles necessary to assist in the safe and effective utilization of drugs by patients
  • Demonstrate knowledge and proficiency regarding special needs and behaviors of age or ethnic specific groups
  • Demonstrated ability to follow written and verbal instructions effectively. 
  • Must have documentation that supports the attainment of the following skill sets:
  • Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
  • Knowledge of patient care charts and patient histories, with the ability to perform basic patient assessments and referrals
  • Ability to maintain quality, safety, and/or infection control standards
  • Knowledge of related accreditation and certification requirements
  • Ability to communicate technical information to non-technical personnel.  Along with the ability to analyze and understand physicians’ or other recognized medical personnel’s oral or written orders involving standard symbols and terminology.
  • American Society of Health-System Pharmacist (ASHP) Board Certification (preferred)
  • Graduate of an accredited College of Pharmacy with a Bachelor’s of Pharmacy.
  • Doctor of Pharmacy (preferred)
  • ASHP accredited Pharmacy Practice Residency (preferred)
  • BCPS (preferred)
  • Licensed as a registered pharmacist in the state of Florida
