Our 224-bed hospital includes a medical ICU, cardiovascular ICU, PCU, open heart surgery center and behavioral health. In addition, we have accredited programs in orthopedics, chest pain, and inpatient diabetes, plus an award-winning emergency department.


Qualifications

You will be responsible for:

  • Performs a variety of general office duties daily.
  • Performs concurrent patient reviews related to appropriateness, level of care, diagnostics and procedures performed.
  • Applies appropriate screening criteria(s) in a timely manner to determined initial admission and continued stay necessity.
  • Initiates appropriate activities when patients do not meet criteria or when alternative care is needed.
  • Communicates findings to insurance companies in a timely and accurate manner.
  • Determines appropriate admission status and communicates with appropriate individuals.
  • Reviews case for appropriate documentation which supports the patients intensity of service and severity of illness.
  • Performs quality indicators reviews.  Screens cases for required monitors.
  • Identifies clinical risk issues and reports to appropriate department.
  • Collaborates with members of the healthcare team in developing discharge plans in a timely manner.
  • Develops discharge plans based upon a family and patient’s needs assessment, capabilities, limitations and availability of community resources.
  • Assists with and arranges referrals and/or transfers to outside agencies/facilities.
  • Participates actively in LOS and Patient Care Conferences.
  • Actively participates in Performance Improvement Teams.
  • Knows the Performance Improvement (PI) Model – the Plan, Do, Check, Act (PDCA) Model.
  • Able to describe process that was improved by using the PDCA Model concepts.
  • Knows the organization-wide Performance Improvement Priorities for the current year.
What will you need?
  • Associate degree or equivalent in health related field.
  • Must have a minimum of 3 continuous years’ experience in an acute care setting, preferably hospital, or at least 2 years’ experience in case management / utilization review with managed care company and 2-3 years’ experience in an acute care setting.
  • Knowledge of medical terminology, disease process and continuum of care for medical-surgical patients.
  • Good organizational skills; uses time efficiently.
  • Team player.
