Qualifications
GENERAL SUMMARY:

The Manager of Ambulatory Care Coordination is responsible for leading and optimizing the performance of the Shawnee Mission Medical Center NurseLine and Shawnee Mission Physicians Group Care Coordination teams through collaboration with internal and external partners, strategic planning, operational oversight and mentoring of associates for success.  This will require a deep understanding of healthcare quality programs, payer/provider partnerships and Care Coordination and Navigation principles, as well as the ability to apply the Whole Care Experience to leadership of associates.  This position has oversight for ambulatory-based Care Coordination and Navigation and Healthcare Quality operations, including but not limited to the Shawnee Mission Physicians Group employed provider practices and Clinically Integrated Network.

 

You will be responsible for:

  1. Responsible for leading and managing practice transformative quality initiatives, such as Comprehensive Primary Care Plus through CMS. This includes maintaining compliance within the specified metrics, reporting necessary data back to requesting entities, hiring and leading additional staff to meet the program initiatives, and staying within budget of provided funding.
  1. Responsible for participating as needed on behalf of SMMC as a representative to any provider partnerships or Clinically Integrated Network.
  1. Lead all assigned teams, acting as day to day operational leader, and performs as needed in the absence of Director including:
    1. Shawnee Mission Physicians Group Care Coordination team: social work, diabetes education, care coordination, care management and quality
    1. Shawnee Mission Medical Center NurseLine team: community triage and care navigation
  1. Work with all hospital and practice associates, providers and leaders as a resource on the coordination and navigation of care through the lifespan, and collaborates to meet educational and business needs
  1. Understands, develops, directs and participates, as needed, with care coordination, chronic care management, care navigation, transitional care management and healthcare quality efforts.
  1. Identifies and maximizes opportunities for reimbursement for care coordination and quality
  1. Maintains prioritization of the functions among direct reports.
  1. Provides regular updates to administration.
  1. Represents Shawnee Mission Health in a positive manner through knowledge, presentation, collegiality and reputation. 
  1. Performs all duties as assigned, including but not limited to what is contained in this job description, within the incumbent’s scope and ability.
 
 

 
What will you need?
  • Bachelor’s Degree in Nursing
  • 2 to 5 Years of Position-Related Experience
  • Kansas Registered Nurse License
