Work Hours/Shift:

You Will Be Responsible For:
  • Coordinates health management workflows ensuring the process is efficient and effective, and that members are referred for appropriate intervention in a timely manner.
  • Monitors caseload reports and provides appropriate feedback to health advisors and department leadership.
  • Ensures that all workflow processes include appropriate and reportable documentation, so outcomes and effectiveness can be measured.
  • Prepares reports/materials and assists with analyzing data trends related to health management outreach and engagement.
  • Collaborates with Health Management Operations and other HM departments to ensure an efficient and consistent workflow between departments and programs.
  • Actively participates in department pilots aimed at increasing Health Advisor efficiency and/or capacity
  • Communicates appropriately and effectively with department leadership, team members and other Health Management department colleagues.
  • Performs other duties as assigned or directed to ensure smooth and efficient operations of the Chronic and Complex Care department.
  • Participates in department process improvement activities, including identifying workflows/processes that are suboptimal and recommending changes.
What You Will Need:
  • Bachelor’s Degree in business or a healthcare related field
  • 3-5 years’ experience in a healthcare setting or operational/support setting
  • Knowledge of healthcare operations
Job Summary:
The Business Workflow Analyst serves Population Health Services Organization (PHSO) Chronic and Complex Care department by ensuring smooth, effective workflows for health management. Coordinates the flow of members to Health Advisors through the utilization of the care management platform, member and provider referrals, interdepartmental referrals, and daily census reports. The Business Workflow Analyst provides daily oversight of processes ensuring that members are delivered to the appropriate Health Advisor in a timely manner, in accordance policy and accreditation standards. The Analyst also ensures that members are identified and referred for health management in accordance with client contractual requirements. Works closely with department director and manager to develop, monitor and improve department processes. Tracks and communicates issues and creates awareness of any potential workflow related risks or problems and recommends options to mitigate. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
