You Will Be Responsible For:
  • Demonstrates good communication skills respectfully and courteous when interacting with patients, co-workers, physicians, and colleagues within department.
  • May round with physicians and Care Management Coordinators to assist with coordination of care and follow up scheduling.
  • Performs front office responsibilities as assigned.
  • Accepts flexibility of schedule to meet program needs.
  • Maintains efficiency in patient work-up flow and triage responsibilities.
  • Communicate information to patients and families and other providers and service representatives.
  • Communicates effectively with front desk staff in practices to ensure efficient patient flow.
  • Demonstrates care for one another by working all areas, open to change, sensitive to department needs, effectively participates in conflict resolution/problem solving.
  • Collaborates with on-call Physicians to monitor utilization of resources to deliver quality of care in a safe, efficient and cost-effective manner
  • Demonstrates knowledge and compliance of Hospital guidelines and regulatory requirements.
  • Improves positive patient satisfaction outcomes by assuring that all quality measures are met in regards to Press Ganey.
  • Participates in ongoing staff development
  • Serves as mentor and advisor to new team members
  • Participates in marketing efforts for promotion of Patient Navigation.
  • Prevents casual overtime at all times and completes work in a timely manner.
  • Maintains accurate legible documentation of patient medications and messages, sample meds, and pharmacy documentation.
  • Keeps current with new technology and updates on care and medications.
  • Schedule follow-up appointment using Physician Group Systems for patients after delivery and during the post op period according to discharge instructions.
  • Obtains discharge summary from all ED and hospitalizations as necessary for physicians.  
  • Contact patient with follow-up call within 2 days of discharge day for high risk patients.
  • Discuss Navigation role to patient and its process.
  • Considers factors related to safety, effectiveness, cost and impact on practice in the delivery of navigating services.
  • Maintains timely communication with patients re: surgery, scripts, testing.
  • Contributes to the growth of the practice by managing patient flow and recommending follow-up visits if needed.
  • Other duties as assigned.

Qualifications
What You Will Need:
 
  • High School diploma/equivalent
  • Graduate from an accredited RN program required
  • Current, valid CPR certification is required (BLS)
 
Job Summary:

The OB Navigator is the main point of contact for all obstetric (OB) patients. They provide appropriate care to patients within the scope of the Medical Practice Act. This individual navigates each OB patient through their entire pregnancy and coordinates care for the OB patient with the hospital navigation staff as well. They will assist in patient education, triage calls, office visits, coordinating classes and registration with the hospital and developing birth plans.
