Hospital Medicine RN - Lead - System Hospitalist - Orlando

AdventHealth Orlando seeks to hire a Hospital Medicine RN - Lead who will embrace our mission to extend the healing ministry of Christ.

Located on a lush tropical campus, our flagship hospital, 1,368-bed AdventHealth Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. We are already one of the busiest hospitals in the nation, providing service excellence to more than 32,000 inpatients and 125,000 outpatients each year.

Job Summary:

The Hospitalist RN-Lead functions in a supportive role to the Executive Medical Director and Hospital Medicine Leadership Team to support the Hospitalist Strategy and delivery of clinical excellence. The Hospitalist RN-Lead partners with the contracted Hospitalist MD (Medical Doctor)/Provider and other Care Team Leaders to achieve success in achieving the process and outcome measures supporting the Hospitalist Scorecard. The Hospitalist RN Lead will partner with Case Management for escalated issues and complex cases in an effort to help facilitate patient throughput and appropriate discharge placement. In addition to general Hospitalist RN duties, the Hospitalist RN-Lead is responsible for orientation and onboarding new Hospitalist RN’s, competencies and setting accountabilities at each campus. The Hospitalist RN-Lead serves as a change agent with the Hospitalist groups and supports conflict resolution. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

  • Bachelor of Science in Nursing
  • Three to five years RN experience
  • Master’s Degree in Business, Nursing, Health Care Administration, or related degree (Preferred)
  • Two to three years of supervisory or management experience (Preferred)
  • Acute care experience (Preferred)
  • Experience in Case Management, Managed Care, Discharge Planning and Utilization Management (Preferred)
  • Case Management training (Preferred)

Licensure, Certification, or Registration Required:

  • Current valid Florida State Licensure as a RN
  • BLS
  • Case Management certification, ACMA (American Case Management Association)/CMSA (Case Management Society of America) (Preferred)

Job Responsibilities:

  • Monitors accountabilities for the Hospitalist RN teams at each campus. Serves as a clinical and educational role model and mentor.
  • Ensures appropriate PDO and holiday coverage to ensure no gap in care processes and communicates any issues to manager.
  • Evaluates supervised employees, recommends promotion of supervised employees, and recommends bonuses and awarding of other discretionary rewards to supervised employees.
  • Reviews census report daily to ensure prioritization of patients based upon system needs.
  • Works with the Hospital Medicine RNs to ensure focus on key metrics to improve performance.
  • Liaises between Care Management and Hospitalist providers, works collaboratively to solve patient challenges which includes, but not limited to, financial barriers, religious beliefs and cognitive issues. Acts as an escalation team for Floor Care Managers.
  • Connects with Hospitalist providers each day to review discharge plans on all patients. Identifies patients that have a discharge delay and identifies barriers to discharge. Works with providers to facilitate safe and appropriate discharges.
  • Manages the orientation and onboarding process for all new Hospital Medicine RN’s to include completing orientation, competencies, and developing additional educational plans as needed.
  • Works across all contracted Hospitalist groups to identify areas of improvement and provides conflict resolution as needed.
  • Collaboratively manages interdisciplinary patient discharge plan. Effectively communicates across continuum of care.
  • Follows-up on any outstanding diagnostic test results, pending consults, patient requests or barriers for discharge. Avoids delay by ensuring test results are available for review and decision making.
  • Participates in outlier meetings as appropriate. Communicates information with providers and facilitates changes in plan of care as directed by physician.
  • Actively participates collaboratively with the Care Management Department in support of system goals for capacity management.
  • Attends Hospitalist Council or other physician meetings to participate in sharing feedback, present findings and assist with problem identification and resolution. Plays an active role in educating resident physicians and Hospitalists on patient care management from a care management discipline.
  • Maintains knowledge of federal and regulatory policies in the patient care management.
  • Uses discretion and adheres to policies and procedures when discussing patient related issues that are confidential in nature. Protects patient privacy with all interactions. Maintains the confidentiality of employees, patients, administrative staff, functions and medical staff information with no infractions.
  • Performs other duties as assigned.
