Care Management Assistant – Care Coordination Center – Maitland

AdventHealth Maitland seeks to hire a Care Management Assistant who will embrace our mission to extend the healing ministry of Christ.



Facility Profile:

Established in 1908, AdventHealth is one of the largest not-for-profit healthcare systems in the country, caring for more than a million patients each year. The Maitland Office Plaza houses our highly skilled teams that support our hospital system including Marketing, Patient Financial Services, Revenue Management, the Credit Union and Human Resources. Department Profile:

The Care Coordination Center is a multifaceted department dedicated to providing patient-centered, relationship-driven care “beyond the four walls” of AdventHealth. The center provides concierge level support to patients, fellow AdventHealth staff members, and community partners. Our team is comprised of both clinical and non-clinical professional team members dedicated to: improving patient throughput, increasing community partnership support, and supporting initiatives to reduce readmissions. We provide service to acute care patients as well as post-discharge and outpatients. We are located at the Orlando campus, which offers an onsite cafeteria, retail stores, and free parking.

Job Summary:

The Care Management Assistant (CMA) is responsible for implementing patient discharge plans as directed, assisting with coordination of clinical data collection, and ensuring third-party payer notifications and authorizations are secured and documented as a support function for Care Management and Care Coordination staff. Interacts with representatives from insurance companies, managed care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies as required. Responsible for assisting in coordinating care and services for assigned patients under the direction of the RN Care Manager and/or Social Worker and in collaboration with the patient/family. The CMA establishes a general working knowledge of payer-specific documentation and authorization requirements, and regulatory requirements regarding the clinical review process, provision of services, and post discharge services. Ensures discharge arrangements are in compliance with payer requirements, and regulatory guidelines. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Job Responsibilities:

  • Implements patient discharge plans prepared by RN Care Manager and/or Social Worker team members
  • Assists Care Managers by performing a variety of tasks as required. These may include but are not limited to:
  • Checking for nursing home, rehab, adult foster care and other facility vacancies/placement
  • Photocopying and faxing appropriate information to above facilities to help facilitate placement as requested by RN Care Managers and/or Social Workers
  • Makes referrals for community agencies
  • Makes home care referrals, ensuring forms are completed correctly and compliantly
  • Explores availability and updating of community resources
  • Assists patients and/or their families/caregivers with concrete needs such as transportation, emergency medications, medical equipment, etc.
  • Assists patients post discharge who have questions related to discharge plan implementation
  • Contacts third-party payers to obtain and/or confirm authorization for services and communicates necessary related clinical information, when required.
  • Assists Care Manager/Care Coordination department in collecting data related to patient-care outcomes and auditing data for accuracy.
  • Prepares documents and reports required data for clinical reviews and discharge plans for third-party payers and other internal and/or external groups.
  • Copies, scans, files, and distributes documents.
  • Documents activities and progress in medical charts per defined policy and procedures, computer billing/utilization systems, care coordination documentation system, or in other ways as directed.
  • Inputs clinical information, and discharge planning details into the medical record.
  • Coordinates appointments with physicians, testing and other services when appropriate. Schedules outpatient follow-up appointments, procedures, and therapies as justified by physician order.
  • Provides verbal and written confirmation of appointments to patients/families. Documents the patients’/families’ agreement and understanding of schedule. Faxes appointment confirmations to the appropriate community provider.
  • Keeps RN Care Manager, Social Worker and/or other members of clinical team informed of issues or delays impacting a timely discharge, or post-discharge scheduling.
  • Issues and documents Medicare or other communication provided to patient/families (e.g., IML for Medicare beneficiaries) as needed.
